Middle ear এর খুব কমন একটা disease, Otitis media সম্পর্কে আমরা ছোটবেলাতেই জেনে এসেছি তাই না?
আজকের Blog টা Otitis media নিয়ে নয়, বরং Otitis media সহ middle ear এর বিভিন্ন infection এর management এ surgical option হিসেবে বহুল প্রচলিত ” Myringotomy ” নিয়ে।

🔴 What is Myringotomy?

👉 It is an incision of tympanic membrane with the purpose to drain suppurative or non-supparative effusion of the middle ear or to provide areation in case of malfunctioning eustatian tube is called myringotomy.

অর্থাৎ, middle ear infection এর কারণে যদি effusion দেখা দেয় এবং eustatian tube malfunctioning থাকে, তখন effusion drain করা এবং eustatian tube এর সাথে ventilation maintain করার জন্য tympanic membrane এ incision ছোট্ট একটা incision দেওয়া হয়।
এটাই myringotomy.

💠 Incision এর পরে drainage টা maintained রাখার উদ্দেশ্য, equalizing tube or tympanoplasty tube insert করা হয় যার আরেক নাম Grommet.

🔶 Site of incision :

1. Antero-inferior (radical)
2. Postero-inferior
3. Antero-superior
   🔵 Most preferable is antero-inferior incision.

🟣 কোন কোন ক্ষেত্রে myringotomy করা হয়?

• 🔺Acute supparative otitis media with severe headache with bulged tympanic membrane.
• 🔺Complications of otitis media e.g. facial palsy, labyrinthitis,meningitis.
• 🔺Acute otitis media not responding to medical treatment.
• 🔺Otitis media with effusion.
• 🔺Recurrent otitis media
• 🔺Baro-traumatic otitis media
• 🔺 Atelectatic ear / Retracted tympanic membrane.

🟦 Myringotomy করা যাবে না কোন কোন ক্ষেত্রে?

1. Otitis externa.
2. Otomycosis.
3. Nasal allergy.

✳️ Myringotomy করলে কি কি complications হতে পারে?

🔺 Injury to incudostapedial joint or stapes
🔺Injury to jugular bulb with profuse bleeding
🔺Injury to chorda tympani nerve
🔺Injury to inner ear
🔺complication of grommet insertion:

• 🔺Tympanosclerosis
• 🔺Retraction pocket formation

• Retraction pocket formation

🟤 Myringotomy তো হয়ে গেলো। এবার patient এর কিছু post operative care নিতে হবে।

1. Daily mopping of ear discharge in case of acute suppurative otitis media.
2. Avoidance of water entry for at least 1 week
3. Coming for follow up in every 3 months.
